Preparing for adulthood

A Preparing for Adulthood assessment helps us understand your adult needs. Independence looks different to every individual. From 13 years old, the main focus will be supporting your independence at your Education Health and Care Planning meeting.

Read our preparing to adulthood pathway guide - for young people with SEND.

You will have a Preparing for Adulthood assessment in the academic year you turn 14 and again in the academic year you turn 16 if you are known to Children and Young People Disability Service (CYPDS).
